These are occurrences that happen in the venue that may have put members of the public or Roxy team members in harms way. See below the method of reporting these:
- Accident Book - on site, there is a designated book to capture all necessary information on what happened, who was involved and contact details for those involved.
- Trail - this needs to be completed by searching ‘Accident + Near Miss’ or ‘Incident’ reporting log. This will ask for all the information gathered along with an image of the incident report book entry, close up and wide angle pictures of the area it occurred, image of the injury (if you have persons consent) and CCTV timestamp for Compliance to find any footage.
'An image of the area' must be of the direct scene where the incident occurred, not via the CCTV screen from the office. This is to spot any immediate faults or issues in the area.

Venue Investigation Report - depending on the severity of the incident that occurred, the GM may be required to complete a venue-level investigation report. Compliance will send over a link to fill out the form. Complete with all available information, respond to the email explaining why any subjects are missing information.
Training
We have an insurance company who deal with claims on our behalf once we have provided then with information, CCTV and witness statements of incidents. They have an accident investigation training course, which is free to complete, that highlights the process of completing an accident/incident report, importance and understanding on gathering information and protecting the public from immediate and future incidents occurring. This course is available to all managers via Mapal. If you do wish to complete this course, the General Manager can access Mapal for their venues team members and management and assign this course:

Accident Reporting Book - this can be purchased through your venue Amazon account. This book is GDPR certified as you can fill out a page, then tear it away from the main book, but you must write in which page insert each one is.
