Mechanical equipment within your venue is dangerous and require specialist training. These machines include Batting Cage, Duck Pin and Ten Pin bowling. 

To protect those who aren't trained to carry out work on the machines without prior training, there needs to be a maintenance sheet on the final exit point when leaving the back area of the machinery. This is to ensure those who have worked on the machinery are trained and working safely. 

These sheets below are to be printed and put into a snap frame. The form will ask for the following:

• Date
• What was the issue
• Parts used
• Lane
• Who (fixed the machine)
• Any injuries to report?
